Strange random pixel on output

I recently output some spots to tape and noticed that I had a random bad pixel. It would show up in different places on a few scenes. The weird thing is it was not visible on the computer monitor of FCP - only on my broadcast monitor. I was able to add a patch and made it go away but it is strange that it was not visible in FCP. Any ideas or suggestions for this?

Kinda hard to suggest anything without info on your system. What is your I/O (Aja Kona, Decklink, etc)? What are your Sequence Settings? Have you tried dumping your Preferences?
