Item Level Render changes clip

Why is it that sometimes (maybe every three days or so) when I do an item level render, an audio clip that's playing is the wrong section? If I re-lay the audio section (by matching clip and replacing) then re-render, the problem goes away.

This can happen with both music aifs and regular captured dailies.

Any ideas?
