Log and transfer issues

2 problems:

1) When I log and transfer footage from 5dMarkII, my file names are changing...(I had to re-log and transfer and it began with the last number from the previous log and transfer)
2) now, my window is stuck, meaning I can't expand it by dragging the bottom right corner to see the secondary window.

Is there a way to reset the log and transfer window (and the numbering?)
